  • First license
  • Expired
  • Supervised Instruction Permit under 17 years of age
  • Medical Caution Notice(re-evaluation) or Skills test requirement(re-occurring
Required Documents
  1. Proof of Identity (photo)
  2. Vehicle Permission: If the vehicle being used is not owned by the applicant, permission must be provided
  3. Vehicle Insurance
  4. Vehicle Registration
  5. Skills Test Receipt
  6. Skills Test Evaluation Form
Things To Know:

  • Appointment is scheduled after payment has bee confirmed. 
  • Cash, Debit and Credit can be accepted. There is a $20 fee when using Debit/ Credit Cards
  • In the event you can not make the appointment or are more than 15 minutes late, no refunds!
